Nurse-Family Partnership

Nurse-Family Partnership  is an evidence- based, community health program that serves serves low-income, pregnant women.

We provide anyone experiencing pregnancy with their own personal registered nurse who will provide support, advice, and wonderful information during pregnancy and after the baby arrives up to 2 years of age. Our nurses will travel to you, so you don’t have to arrange transportation.

During visits, nurse home visitors will educate on personal and environmental health (both during pregnancy and after delivery), infant/toddler health and development, and parenting skills. Parents will also receive support and education on meeting life goals, and will be referred to community agencies, as needed.

Information

Eligibility

You must be experiencing pregnancy at 28 weeks or less and reside in one of the 13 counties that we serve in Northeast Tennessee: Carter, Claiborne, Cocke, Grainger, Greene, Hamblen, Hancock, Hawkins, Jefferson, Johnson, Sullivan, Unicoi, and Washington. There is also an income requirement that is required to be eligible for the program.

Services
