Donation of Human Remains
The ETSU Quillen College of Medicine appreciates your desire to donate your remains for medical education. To donate your remains, please fill out the enclosed form in the presence of two (2) witnesses. Be sure to include the current date, your birth date and your social security number on the form. Mail the white copy to: Anatomical Gift Program, ETSU Quillen College of Medicine, P.O. Box 70582, Johnson City TN 37614-1708. The colored copy should be given to a relative, friend, or physician with instructions to report your death promptly to the Anatomical Gift Program at 1-866-968-3668 (1-866-YOU-DON8).

- Fill out the entire form in the presence of two (2) witnesses.
- Be sure to include the current date, your birthdate, and your social security number on the form.
- Make a copy of the form for your records.

The Quillen College of Medicine does not pay individuals for donating their remains. It is unlawful to exchange money for anatomical donations. However, the Anatomical Gift Program does arrange for and pay for the expenses of embalming and delivery of your remains to the ETSU College of Medicine if death occurs within a 30-mile radius of Johnson City, Tenn.
Please note that the Anatomical Gift Program will be unable to accept your remains if:
- THE REMAINS ARE NOT RECEIVED WITHIN FIVE (5) HOURS OF DEATH
- an autopsy has been performed,
- any organs have been donated
- the remains exceed 230 pounds,
- you are affected by a contagious disease, such as AIDS, MRSA, VRE, hepatitis, staph)
- when the tissues have been damaged excessively (as in a severe automobile accident) or embalming has been done without approval from our department,

The ETSU Quillen College of Medicine appreciates your generous spirit in this matter. Please note that all utilization and cremation of donated remains is performed locally.
