How to Apply
Eligibility
Thank you for your interest. We are currently accepting applications for 2023-2024 as of July 2022 through ERAS. Applicants currently must be board certified or board eligible in Anesthesiology, Emergency Medicine, Family Medicine, Internal Medicine, Obstetrics and Gynecology, Pediatrics, Preventative Medicine, or Psychiatry. Additionally, applicants must be graduates of, or in the final year of an ACGME accredited clinical residency program.
Application
For Academic Year 2023 we will be accepting applications through ERAS starting July 6th, 2022. All interviews will be virtual.
We are participating in National Resident Match Program (NRMP) Specialties Matching Service this year. Our program number is 2066404F0. NRMP is specific to ACGME-accredited fellowship programs. Addiction Medicine will participate in the Medicine and Pediatric Specialties Match for 2023 appointment year. Match Day will be held on November 30, 2022
The Department of Family Medicine only accepts applications through the Electronic Residency Application Service (ERAS).

Information about the service may be obtained from your Dean's Office or from the American Association of Medical Colleges (AAMC) International medical graduates may obtain information about ERAS through the Educational Commission for Foreign Medical Graduates (ECFMG) We welcome ECFMG sponsored J-1 visas.
We Request
We ask that in addition to your ERAS application that you please also submit your
Curriculum Vitae, Personal Statement and three (3) letters of recommendation. The
one to two page personal statement should describe your interest in Addiction Medicine and your career goals upon fellowship
completion. Please address:
- What prior experiences you have had providing medical care to persons with unhealthy substance use and persons with health disparities from rural, low-income, or stigmatized populations. How have these experiences influenced your life and career?
- Describe your interest in addiction medicine and any specific areas that you would like to focus on clinically during your fellowship training.
- What are your overall career goals? Describe what you would like to be doing five to ten years from now. How do you anticipate this fellowship will assist in your plan?
If you are a physician with a demonstrated commitment to a career integrating evidence-based addiction medicine with care of the rural underserved, we hope you will apply with confidence.
