The Center for Cardiovascular Risk Research has partnered with the University of Tennessee Health Science Center, the University of Memphis, and Qsource to create a statewide network of primary care providers, health systems, and academic centers committed to identifying, developing, and implementing patient-centered approaches to improve the quality of care and health outcomes for Tennesseans with heart disease. 75 patient care providers across the state will participate in the study, with 25 facilities from upper East Tennessee. Each patient care facility participating in the Tennessee Heart Health Network (TNHHN) will choose up to 3 patient-centered toolkits:
