During the verification process, the college financial aid administrator will request the applicant to supply copies of documentation, such as a verification worksheet, tax return transcripts (obtained from the IRS), W-2 statements and 1099 forms, in order to verify the data that was submitted on the Free Application for Federal Student Aid (FAFSA).  Additional documentation may be requested if conflicting data is found between the FAFSA and the documentation provided.  Requested documentation is necessary to complete the verification process in order to award the student financial aid. 

If you are chosen at random for verification by the Federal Processor, you must submit additional paperwork to the Office of Financial Aid before your application is considered complete. If you are selected for verification and do not complete this verification process, you will not receive any financial aid.  

How do I know if I have been selected for verification? 

  1. Check the FAFSA website 3-5 days after completion of FAFSA or after each FAFSA correction. 
  2. Find and read the SAR(Student Aid Report). The SAR will tell you if any additional information is required, or if you have been selected for verification 
  • Login to the FAFSA website 
  • Make sure the " Current Application Status" says " Processed Successfully" 
  • Proceed to " View Processed Information" 
  • Confirm your identity with your Federal FAFSA PIN 
  • Select the most recent transaction 
  • Select the " SAR Comments" link on the left and read carefully to see if your application has been selected for verification. 

  

What do I do if I have been selected for verification? 

Students selected for verification by the US Department of Education are required to submit copies of documentation including, but not limited to, verification worksheets, tax return transcripts (obtained from the IRS), W-2 and/or 1099 forms, in order to verify the information that was submitted on the FAFSA. Additional documentation may be requested if the information submitted on the FAFSA does not match the documentation provided. Requested documentation is necessary to complete the verification process in order to award financial aid to students. The required institutional documents are to be submitted online. Detailed instructions on how to complete the forms for both students and parents or legal guardian below step 6.

  1. If you are selected for verification, submit all required verification documents.

    In order to reduce the burden placed on students and parents related to the verification process, on January 9, 2019, the U.S. Department of Education released new guidance to colleges and universities. Specifically, students and parents may now submit, to the Financial Aid Office, signed copies of tax returns in lieu of Tax Return Transcripts from the IRS.

    *If the individual does not have a copy of the signed Tax Return, they will still be required to submit the Tax Return Transcript.
  2. The required documents will include some worksheets and a Federal tax return transcript from the IRS.  The transcript can be ordered online, by mail or over the phone. In order to avoid ordering a tax return transcript from the IRS, we recommend using the IRS Data Retrieval Tool when completing your FAFSA. 
  3. You can get various transcript typesonline or through the mail. The method you used to file your return and whether you have a refund or balance due, affects when a current year transcript is available.  
  4. Be sure to request a Tax Return Transcript. DO NOT order a Tax Account Transcript unless specifically requested by the Office of Financial Aid. If you do not have copies of ALL of your W2 forms, you will also need to request a Wage and Income Transcript.These transcripts are mailed to your home address, free of charge. Allow 5 to 10 business days for delivery. 
  5. Check your student requirements on GoldLink frequently. 
  6. Respond immediately to any and all requests from the Office of Financial Aid for information. Check Email daily. 

If the parent, student, or spouse attempts but is unable to obtain the verification of non-filing from the IRS and the school has no reason to question the individual’s good-faith effort to obtain the required documentation, the school may accept as a replacement for the verification/confirmation of non-filing:

  1. A signed statement certifying that the individual:
    • Attempted to obtain the verification or confirmation of non-filing from the IRS and was unable to obtain it, and
    • Has not filed and is not required to file an income tax return for the tax year being verified; and
    • List the sources and amounts from each source of any income earned from work by the individual for the tax year being verified; and
  2. A copy of IRS Form W-2, or an equivalent document, for each source of employment income received by the individual for the tax year being verified. 

NOTE: W-2 forms (due to COVID-19, this change is effective beginning 4/20/2020 and only valid until such time as the IRS begins accepting paper requests for transcripts, form 4508-T):

If an individual who is required to submit an IRS Form W-2 is unable to obtain a Wage Transcript from the IRS in a timely manner, he/she can provide a signed statement that includes:

  • the amount of income earned from work, and
  • the source of that income, and
  • the reason why the W-2 is not available in a timely manner

If there is a discrepancy between the amount of wages earned in the signed statement and the amount of wages earned on the IRS Tax Return Transcript, the individual will be required to obtain a Wage Transcript, even if he/she has to wait until such time as the IRS begins accepting paper 4508-T forms. When the IRS returns to accepting paper 4508-T forms, we will require the form W-2 from IRS and no longer accept signed statements.

Students

Note: This guide is only for ETSU Students, Faculty and Staff will log into Dynamic Forms with their GoldLink username and password.
**Dependent Students ONLY PLEASE NOTE** The Office of Financial Aid will NOT be able to receive or view your form until the parent sections have been completed and submitted by the parent you have designated.

Login to Goldlink

If you do not yet have a GoldLink account, you will need to activate your student account before being able to access and complete the verification form.

  1. Once logged into GoldLink Select the “Financial Aid” Page.
  2. Click to open the “Financial Aid Requirements” pane.

Click on a Form Link to Login to Dynamic Forms

You will login to your dynamic form using your ETSU username and password.

  1. Click on the Link(s) under the Requirements section.
  2. Enter your username and password and click Submit

Complete Your Dynamic Form

You will login to your dynamic form using your ETSU username and password.

  1. Click the blue “Complete This Form” button on the following page.

    Dependent Students ONLY 
  2. Enter your parent’s information by clicking “Enter contact information” and click “Save” – The parent information entered here should be a parent listed on your FAFSA.
  3. Select “Continue to form” to progress to your form.
  4. Complete the student information sections with accurate information and click “Next” to continue.

Sign Your Form

On the following page type your name into the Electronic Signature fields and click the blue “Sign Electronically” button to submit your e-signed form.

If successful, you will receive a Thank You Message once your form has been submitted and options to view your submitted form as a PDF or to Logout.

Save your Progress

If you are unable to complete the form in one sitting you may save your progress at any time and return to your form in progress at a later date. To do this:

  1. Click on the blue “Save Progress” button at the bottom of the form.
  2. When you are able to continue your form login to Dynamic Forms by following the link in the original email you received or
  3. You may also login to Dynamic Forms here https://dynamicforms.ngwebsolutions.com and clicking on the “Pending / Drafts Forms “button. From the list of Pending / Drafts forms click on the “Complete this form” next to the form you would like to complete.

Fix Rejected Form

If your parent observes that any of the information you have entered is incorrect or missing, they have an option to reject your form and send it back to you for corrections and resubmission. If this occurs, you will be sent an email regarding the error with a link to update your form.

  1. Read the rejection email thoroughly for information about what needs to be corrected on your form.
  2. Click the link provided in your email labeled “Click here to complete your section of the form”.
  3. Fix incorrect or missing information and resubmit your form.

Duplicate Submission Request

If you attempt to fill out a form more than once, you will receive a warning letting you know that you have already successfully submitted the form once.

Parents/Legal Guardian

This is only for parents creating a Dynamic Forms account and returning parent users. Students will log into Dynamic Forms with their Goldlink username and password.

Create an Account:

If you do not have a Dynamic Forms account, you will need to create an account before signing your student’s form. To create your free account:

  1. Click on the link provided in the email from forms@etsu.edu “Click here to complete your section of the form.”
  2. From the Log In page select "Create a new Account" located beleow the blue "Login" button. 
  3. Complete the “Create a new account” form using a non etsu.edu email address and click the “Create Account” button at the bottom of the form.
  4. Activate Your New Account by logging into the email account you used to create your Dynamic Forms account, opening the verify your account email from forms@etsu.edu, and clicking on the “Activate your account” link. You will receive an Account Activated message if successful.
  5. Click the “Continue” button to login to Dynamic Forms and proceed to your form(s). Continue with the following section of this guide.

Complete your Form

Once you have a Dynamic Forms account:

  1. Click on the link provided in the email from forms@etsu.edu requesting your e-signature on a form - “Click here to complete your section of the form” and login to Dynamic Forms
  2. Click the blue “Complete This Form” button on the following page.
  3. Complete the parent information section(s) of your form and click the blue “Next” button at the bottom of the form to continue with the form submission
  4. On the following page type your name into the Electronic Signature fields and click the blue “Sign Electronically” button to submit your e-signed form. If successful you will receive a Thank You! Message once your form has been submitted and options to view your submitted form as a PDF or to Logout.

Save your Progress

If you are unable to complete the form in one sitting you may save your progress at any time and return to your form in progress at a later date. To do this:

  1. Click on the blue “Save Progress” button at the bottom of the form.
  2. When you are able to continue your form login to Dynamic Forms by following the link in the original email you received or
  3. You may also login to Dynamic Forms here https://dynamicforms.ngwebsolutions.com and clicking on the “Pending / Drafts Forms “ button. From the list of Pending / Drafts forms click on the “Complete this form” next to the form you would like to complete.

Reject Incorrect Form

If you find incorrect information your student has entered, you can “reject” your form sending it back to your student for corrections. To do this:

  1. Click on the “Reject” button at the bottom of your form.
  2. On the next page fill out the email subject and body with information about what information is incorrect and should be corrected.
  3. Click the red “Reject this Form” button at the bottom of this page to send your email along with a link to the form to your student.
  4. Once your student has corrected the misinformation and resubmitted the form you will receive another email requesting your signature on the form with a link to the updated form. Use this link to complete and sign your form.

Duplicate Submission Request

If your student attempts to fill out the form more than once they will receive a warning letting them know that they have already successfully submitted the form once.

Is verification required if I ONLY want to apply for the Lottery Scholarship? 

Students who are eligible for Tennessee Educational Lottery Scholarships may be flagged for verification. These students have two options: 

  • Complete the verification process in order to determine if the student may be eligible for any other aid, including grants, loans, and federal work study, OR 
  • The student can complete the TELS Verification Opt-Out form if the student wants NO other aid. This form is available by contacting a Financial Aid Counselor. *NOTE: Only complete this form if your FAFSA was selected for verification. By completing this form, you are stating that you DO NOT want to be awarded any aid other than the Tennessee Hope and/or Merit Scholarship. (You must meet eligibility requirements to receive these awards.) 

 

What if I have NOT been selected for verification at this time? 

Keep checking your financial aid status on GoldLink and your GoldMail frequently to be sure that you have not been selected. 

1. You may be selected for verification at a later time (even after awards have been made) in some circumstances: 

  • Any subsequent corrections to your FAFSA can result in selection for verification. 
  • If at any time conflict arises between your FAFSA report and other information received by the ETSU Office of Financial Aid, your application may be selected for verification. 

2. You may be required to submit additional documentation even if your application is not selected for verification. 

  • In various situations, students may be required to submit certain documents even if they were not selected for verification. Documentation of the following may be required:  
     
     
  • Receipt of food stamps 
  • Legal Guardianship 
  • Orphan, Ward of the Court, or Foster Care 
  • Emancipated Minor 
  • Youth At-Risk of Homelessness 
  • DD  Form214/Veteran status 
  • Selective Service registration 
  • Citizenship status 
  • Resolution of default on federal loans 
  • Discharge of federal loans 
  • Bankruptcy status 
  • Other 

Otherwise, continue to check your status and accept awards. 

Response to COVID19
Submission of Citizenship Documents for Federal Financial Aid
Effective: April 28, 2020 

This emergency submission accommodation goes into effect on April 28, 2020 and will be active until such time as the Office of Financial Aid and Scholarships resumes on campus operations. 

A student who is required to submit proof of citizenship or eligible non-citizen status is required to submit documents to verify their status in person.  Due to COVID19 pandemic and the remote operations of the Office of Financial Aid and Scholarships, students are unable to submit documentation in person. Beginning April 28, 2020, the following submission exception will be effect until such time as the ETSU Office of Financial Aid and Scholarships resumes on campus operations. 

Students can submit the following to the ETSU Office of Financial Aid via postal mail, fax, or secure dropbox link: 

  • A photocopy or scan their citizenship documents (front and back)
  • A photocopy or scan of a valid government issued photo ID (front and back)
  • A signed statement attesting that these are true and exact complete copies of these documents 

If the ETSU Office of Financial Aid and Scholarships questions the validity of the documents submitted, the student will be required to provide the original document for review, as certified by the issuing authority, at such time the office resumes on campus operations.

  

Important things to remember: 

The priority deadlines to have both your FAFSA processed and your file complete are as follows: 

  1. April 15th for FALL term 
  1. November 15th for SPRING term 
  1. March 1st for SUMMER term 

Submitted documents are processed in the date order received.
